I acknowledge that Fearless Femme Beauty Company has implemented health and safety measures to reduce the spread of COVID-19, including but not limited to:
Wearing masks for very close proximity services such as lashes and brow/facial hair removal services and gloves for services that require touch.
Regular sanitation of all equipment and high-touch surfaces.
I agree to comply with all health and safety protocols established by FFBC, including wearing a mask (if requested), and adhering to sanitation guidelines during my visit.
I understand that despite all preventive measures, there is an inherent risk of exposure to COVID-19 and other contagious illnesses when visiting public places, including salons.
I, on behalf of myself and my heirs, hereby release, waive, and discharge FFBC, its owners, employees, and agents from any and all liabilities, claims, actions, or demands arising from or related to exposure to COVID-19 or other contagious illness during my visit.
I agree to inform FFBC if I experience any symptoms of illness or learn of potential exposure to COVID-19 or other contagious illness within 14 days prior and following my appointment.
I understand that Fearless Femme Beauty Company reserves the right to refuse service to anyone exhibiting symptoms of illness or not adhering to health and safety protocols.
